Job Description

The Transitions Case Manager Licensed Clinical Social Worker (TCM-LCSW) works in a busy environment with a responsibility to communicate with patients, health plans and medical groups. Will work directly with patients and key personnel at each ministry, Care Management, insurance companies, and with our community partners, as needed.

Required Qualifications:

  • Master’s Degree in Social Work
  • California Clinical Social Worker License upon hire. Or
  • California Marriage and Family Therapist License upon hire.
  • 5 years of previous experience working in a healthcare role is required.
  • Practical experience in customer service.

Preferred Qualifications:

  • Background in managed care health insurance or care management

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
