Not fully remote, significant in‑person presence required in the community, some administrative tasks (documentation, planning, meetings) can be done remotely
Works across network of congregations and faith-based organizations to build relationships and social cohesion across the community. Brings community members together around common concerns, ensures alignment and linkages across partners, works with the faith community nurse or other Advocate Aurora staff or partners to link community residents with services and advocates for Policy, System and Environmental change strategies to address the social determinants of health and to promote equity and justice in community resources. Works with Faith Community Nurse or other Advocate Aurora Health staff to mobilize support for populations of patients identified as vulnerable or in particular need of faith-rooted care.
Typically requires 3 years of experience in community organizing social work, sociology, political science, or ministry.

- Advanced training beyond High School that includes the completion of an accredited or approved program in Community Health, or
- Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED), or
- High School Graduate
Engage closely with community members and stakeholders to listen, build trust, and understand local needs. Represent the organization within community and faith-based networks, communicating concerns and promoting health-focused programs. Strengthen collaboration across partners, supports community health assessments and planning, and connects residents to services and resources—especially vulnerable populations. Provide social support, coordinates care networks, and advocate for equitable access, funding, and policy changes to address root causes of community health issues.
