Mentors stroke program coordinators in certification and assists in implementation of regional stroke program initiatives and goals.
Involved in interviewing, hiring, and annual performance reviews of stroke coordinators and staff members of the program.
Develops, evaluates and maintains a comprehensive program that meets and exceeds guidelines established by federal regulations and national association standards.
Responsible for ongoing program management across the continuum, from the EMS and hospital entry to return to home, ensuring compliance with recognized standards of care and achieving operational efficiency, patient satisfaction, quality outcome, and optional performance goals. Identifying and implementing patient care processes and protocols to promote appropriate utilization of resources.
Responsible for developing and implementing, in collaboration with Administrative and Medical Director, the short and long-term goals of the program. Ensures that goals are coordinated with the programs strategic plan and is aligned with overall medical center priorities.
Facilitates and monitors quality, financial, operational, and process data to assess and ensure ongoing program development. Responsible for developing, implementing and documenting action plans to address any variation in program metrics. Reviews action plans with the Regional Stroke Program Manager and Medical Director on a quarterly basis.
Participates in the care of patients when possible, participating in rounds and team meetings to maintain skills and identify barriers and needs.
Serves as a clinical expert and resource assisting with care for the complex and/or highly specialized patient care situations, seeking processes to reduce length of stay, cost per patient day, and other patient care efficiencies.
Establishes effective networks with colleagues throughout the Medical Center and referral region. Acts as a resource/liaison with medical staff, patients, families, other departments and the community.
Participates in identifying needs, planning, developing and implementing educational programs within specialty areas for patients, families and/or community groups to improve the care of patients. Monitors effectiveness of programs, making changes as needed.
Promotes and develops personal and professional growth, maintaining professional and clinical knowledge; attending continuing education and in-services as needed. Maintains professional and clinical knowledge, acting as a resource to others.
