ABOUT THE ROLE:
Cares for Community Health patients (adult and pediatric) who present to the emergency department for either substance use and/or behavioral health reasons and addresses their social determinants of health needs. The Behavioral Health Transition Liaison will also assist in the management and coordination of patients that need support in establishing services with a primary care provider and assists the patient in discharge/follow-up planning. The Behavioral Health Transition Liaison will monitor and assist patients for 30 days after discharge from PSIU, psychiatric/addiction facilities, or emergency department.
FUNCTIONS OF THE POSITION:
- Exhibit service excellence standards of behavior at all times.
- Must have the ability and knowledge to collect specimens and follow instructions properly.
- Display knowledge of Quality Control, how to perform QC and Proficiency testing.
- Demonstrates competency in EMR operations, create a requisition, print provider orders, order venipuncture, sign off patient charts, result and/or merging final patient’s results, schedule patients.
- Demonstrates competency in all phlebotomy skills, determine appropriate tubes necessary for designated tests.
- Demonstrates competency in LabTrak ® operations, create a sample label, resulting, sending results to EMR, proper identification, and processing.
- Know how to correctly operate the Sysmex KX-21 ® CBC, Siemens DCA Vantage ® (A1C), Clinitek Status ® (urine) machines and to use the microscope.
- Obtain EKGs
- Correctly run patient’s samples with the above stated machines.
- Assist with the operation of new employees
- Appropriately handles “critical” and “abnormal” patient values
- Maintains appropriate inventory levels in the blood collection area
- Punctual in attendance and wears appropriate attire as required
- Assist providers with any questions
- Courteous and effective communication with patients, staff and providers
- Contacts patients for recollection (if necessary)
- Maintains clean work environment (draw room/testing area)
- Practices universal precautions

SKILLS REQUIRED FOR SUCCESS:
- Office generalist responsibilities require H.S. graduation or equivalent.
- Must pass Phlebotomy Competency, which consists of 10 successful venipunctures.
- CPR certification required.
