About the Role

The Community Health Nursing Home Care Coordinator will assist the providers in managing an optimal workflow by providing a variety of nursing and administrative duties to provide safe and efficient care of the patients.

  • Communicates with provider about any new orders on patients and communicates to the appropriate SNF nurse that there are new orders placed, the indication for those orders while fielding subsequent calls to answer all questions from the nursing staff.
  • Assists in phone outreach to include by not limited to handling caller needs/ directing to appropriate staff, triage calls which will be based on urgency and severity of symptoms and refers to provider for complicated issues.  
  • Obtains and records vital signs utilizing multiple electronic health records.
  • Pre-loads all new patients for admissions and updates current Community Health patient records.
  • Documents and captures all pertinent nursing home data for all Community Health measures.
  • Maintains, tracks, and schedules all regulatory visits.
  • Addresses daily summary action report to ensure Community Health patient records are correct and update to date with new payor sources. 
  • CCMA Preferred, Valid CPR Certification.
  • Prior experience working in a nursing home preferred.
  • Experience in using a variety of electronic medical record and ability to learn other systems, basic keyboarding skills and email communication.
