Hospice Admissions Nurse- RN

Capital City Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our Hospice Admissions team. This role is responsible for conducting pre-admission assessments, educating patients and families, and initiating hospice care services. The ideal candidate is patient-focused, clinically strong, and committed to delivering high-quality end-of-life care.
Responsibilities
~1 min read- →Perform pre-admission evaluations to determine eligibility and appropriateness for hospice services
- →Educate patients and families on hospice philosophy, care options, and services
- →Initiate admission process and obtain informed consent
- →Conduct comprehensive nursing assessments
- →Develop and implement individualized patient-centered care plans
- →Provide interventions that support comfort, dignity, and quality of life
- →Collaborate with interdisciplinary care team
- →Ensure compliance with quality standards and regulatory requirements
Requirements
~1 min read- Graduate of an accredited Registered Nursing (RN) program
- Active Registered Nurse (RN) license in the state of practice
- Current CPR certification
- Ability to make independent clinical decisions
- Strong communication and interpersonal skills
- Ability to work effectively with patients, families, and healthcare professionals
- Knowledge of quality assurance and performance improvement processes
- Minimum 1 year of hospice or home health experience
- Experience with patient admissions and care planning
