Hospice Licensed Practical Nurse Continuous Care

Queen City Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide direct patient care in a hospice setting. This role focuses on delivering high-quality, patient-centered care that supports comfort, dignity, and quality of life for patients and their families. The ideal candidate has strong clinical skills, excellent communication abilities, and a passion for end-of-life care.
Responsibilities
~1 min read- →Provide direct patient care in accordance with the State Nurse Practice Act
- →Deliver nursing care based on comprehensive patient assessments and established plan of care
- →Accurately document patient care and services in a timely manner
- →Educate patients and families on diagnosis, care plans, and symptom management
- →Coordinate hospice services with the interdisciplinary care team
- →Communicate patient needs, status changes, and care updates effectively
- →Provide pain and symptom management interventions
- →Monitor and evaluate patient responses to treatments and medications
Requirements
~1 min read- Graduate of an accredited Practical Nursing (LPN/LVN) or Vocational Nursing program
- Current Licensed Practical Nurse (LPN/LVN) license in the state of practice
- Minimum 3 years of nursing experience
- Current CPR certification
- Strong verbal and written communication skills
- Excellent organizational and time management skills
- Valid driver’s license and reliable, insured vehicle
- Experience in hospice, home health, community health, or medical-surgical nursing
- Understanding of hospice philosophy and end-of-life care principles
