Queen City Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide direct patient care in a hospice setting. Responsibilities

→Provide direct patient care in accordance with the State Nurse Practice Act
→Deliver nursing care based on comprehensive patient assessments and established plan of care
→Accurately document patient care and services in a timely manner
→Educate patients and families on diagnosis, care plans, and symptom management
→Coordinate hospice services with the interdisciplinary care team
→Communicate patient needs, status changes, and care updates effectively
→Provide pain and symptom management interventions
→Monitor and evaluate patient responses to treatments and medications

Requirements

Graduate of an accredited Practical Nursing (LPN/LVN) or Vocational Nursing program
Current Licensed Practical Nurse (LPN/LVN) license in the state of practice
Minimum 3 years of nursing experience
Current CPR certification
Strong verbal and written communication skills
Excellent organizational and time management skills
Valid driver’s license and reliable, insured vehicle

Experience in hospice, home health, community health, or medical-surgical nursing
Understanding of hospice philosophy and end-of-life care principles
