Responsibilities

  • Provide direct patient care in accordance with State Nurse Practice Act and hospice standards
  • Perform comprehensive patient assessments and support individualized plans of care
  • Deliver skilled nursing services focused on comfort and quality of life
  • Administer and monitor pain and symptom management interventions
  • Educate patients and families on diagnoses, disease progression, and care plans
  • Document patient care accurately and timely in the electronic medical record (EMR)
  • Collaborate with the interdisciplinary care team to ensure coordinated hospice care
  • Evaluate patient response to treatment and adjust care as needed

Requirements

  • Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
  • Current LPN/LVN license in the state of practice
  • Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
  • Current BLS certification
  • Strong clinical assessment, communication, and organizational skills
  • Valid driver’s license, reliable transportation, and proof of insurance
  • Understanding of hospice philosophy and end-of-life care

  • Experience in hospice or palliative care
  • Familiarity with home-based care and EMR systems
