Schedule:
Monday–Friday, 8:30 AM–5:00 PM
Every 12th weekend (Saturday and Sunday)

Territory: Crystal Lake, IL

Compensation:
Up to $97,000 annually
Final offer based on years of experience

Responsibilities

→Serve as the primary nurse for assigned patients and families, including assessment, eligibility determination, admissions, care planning, implementation, and evaluation
→Assess physical, psychological, social, and spiritual needs to ensure holistic, patient-centered care
→Collaborate with physicians, interdisciplinary care team members, and community partners to coordinate optimal care
→Conduct regular patient visits to reassess needs and update care plans
→Perform clinical pronouncement of death and provide compassionate support to families
→Maintain timely, accurate patient documentation including charts, progress notes, and care plans
→Supervise and document care provided by Home Health Aides in compliance with federal, state, and company guidelines
