Hospice RN Case Manager

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Crystal Lake area.

• Consistent Monday–Friday schedule
• Only 1 weekend every 12 weeks (significantly less than many competitors)
• Focus on quality of care over visit volume
• Supportive leadership and interdisciplinary team
• Autonomy in the field without feeling isolated

• Monday–Friday, 8:30 AM – 5:00 PM
• One weekend rotation every 12 weeks
• Territory: Crystal Lake, IL and surrounding communities

• →Manage a dedicated hospice caseload as the primary RN Case Manager
• →Perform patient assessments, admissions, and individualized care planning
• →Deliver skilled nursing care focused on comfort and dignity
• →Coordinate care with physicians and interdisciplinary team members
• →Conduct routine visits and adjust care plans as needed
• →Provide end-of-life support, including family education and pronouncement
• →Maintain timely, accurate documentation
• →Supervise Home Health Aides in compliance with regulations

• Graduate of an accredited RN program
• Active Illinois RN license
• Current CPR and First Aid certification
• Hospice or home health experience preferred
• Strong clinical judgment and communication skills
• Valid driver’s license and proof of insurance
