Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Only 2 holiday on-call days per year)

~1 min readAs a Hospice RN Case Manager, you will:
- →Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
- →Deliver holistic patient care, addressing physical, emotional, psychological, social, and spiritual needs
- →Collaborate with the interdisciplinary care team, including physicians, social workers, and chaplains
- →Coordinate care with attending physicians and external providers
- →Conduct routine patient visits and update individualized plans of care
- →Provide end-of-life care, including clinical pronouncement and family support
- →Maintain accurate, timely documentation in compliance with state, federal, and company standards
- →Supervise and oversee care delivery by Home Health Aides (HHAs)
Requirements
~1 min read- Active Registered Nurse (RN) license in Ohio (or compact state if applicable)
- Graduate of an accredited nursing program
- Current CPR and First Aid certification
- Knowledge of quality assurance and hospice care standards
- Valid driver’s license and auto insurance
