What You’ll Do
As a Home Health LPN, you will provide compassionate, hands-on care while collaborating closely with the Registered Nurse and interdisciplinary care team.
Your responsibilities will include:

Providing direct patient care in accordance with the state Nurse Practice Act
-
Carrying out individualized plans of care developed by the Registered Nurse
-
Educating patients and families on treatments, medications, and self-care
-
Completing accurate and timely clinical documentation
-
Coordinating with the RN and care team to ensure comprehensive patient support
-
Following proper procedures for equipment, supplies, and infection control
-
Supporting patients’ physical, emotional, and educational needs in the home
-
Communicating changes in a patient’s condition to the appropriate clinical team members
What You’ll Need
-
Active LPN or LVN license in the state of Tennessee
-
Graduate of an accredited practical or vocational nursing program
-
Three or more years of nursing experience preferred
-
Home Health, Medical-Surgical, or Community Health experience preferred
-
Current CPR certification
-
Reliable transportation
-
Valid driver’s license and current automobile insurance
