Home Health Licensed Practical Nurse (LPN) Full-Time

• Pay: $26.63 - $ 27.96 per hour
• Location: Palm Bay (Brevard Co.)
• Schedule: full-time

• Mileage: Currently $0.60 per mile
• Reports to: Branch Director

• →Provide direct patient care as defined in the state Nurse Practice Act
• →Carry out the plan of care set by the registered nurse
• →Document care accurately and on time
• →Teach patients and families about their diagnosis and care plan
• →Assist the RN or physician with specialized procedures, including equipment prep, aseptic technique, and self-care training
• →Report needs for HHA, OT, PT, MSW, ST, or Dietician services to the RN or Clinical Supervisor
• →Use equipment and supplies efficiently
• →Take on other duties as assigned by the registered nurse

1. Graduate of an accredited practical nurse or vocational nursing program.
2. Preferred: Has three (3) years nursing experience. Community/home health or medical/surgical experience.
3. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
4. Complies with accepted professional standards and practice.
5. Demonstrates good verbal and written communication, and organization skills.
6. Possesses and maintains CPR certification.
7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

This role requires the following, with or without reasonable accommodation:

• Prolonged walking and standing
• Lifting and carrying up to 25 lbs frequently and up to 50 lbs occasionally
• Lifting, positioning, and transferring patients (with assistance for patients over 50 lbs, per agency safe-handling policy)
• Reaching, stooping, bending, kneeling, and crouching
• Visual acuity and hearing sufficient to perform nursing duties

Required before providing patient care:

• Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease
• Documented negative TB test (TST or TB blood test) taken within the past 12 months
• TB risk assessment and symptom evaluation upon hire
