Sign On Bonus: Up to $10,000

Schedule:  Full Time

Shift: Day Shift- Rotating Weekends. 4- 10 hour shifts

Location: This is a fieldwork position in homes and hospitals in Volusia and Flagler Counties.
Travels to patients' locations, including resident sites and hospital rooms, to assess eligibility for Hospice care.
Partners with hospital Case Management staff to meet patient and hospital staff needs and gain new referrals.
Assesses the emotional condition of patients and families to determine readiness for Hospice and address questions and concerns.
Coordinates patient treatment efforts and promote quality improvement and educational initiatives.
Obtains required signatures and generate necessary electronic or paper documents for regulatory compliance and reimbursement.

Knowledge, Skills, and Abilities:
• Demonstrates ability to communicate by reading, writing, and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
• Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]
• Demonstrates basic computer skills. [Required]
• Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form. [Required]
• Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]
• Ability to complete AdventHealth Hospice Care Orientation and Training, as necessary. [Required]

Education:
• Associate's of Nursing [Required]
• Bachelor's of Nursing [Preferred]

Field of Study:
• N/A

Work Experience:
• 1+ years Registered Nurse in acute care nursing experience [Required]
• Hospice experience [Preferred]

Additional Information:
• N/A

Licenses and Certifications:
• Basic Life Support - CPR Cert (BLS) [Required]
• Registered Nurse (RN) [Required]
• Driver's License (DL) [Required]
