We're hiring compassionate Home Health Medical Social Workers (MSWs) in Oakland, Macomb and Washtenaw counties to provide personalized, one-on-one support that helps patients and families navigate life's challenges, access essential resources, and remain safe and independent at home. Responsibilities
~1 min read- →Conduct comprehensive psychosocial assessments to identify patient and family needs, develop individualized care plans, provide interventions, and monitor progress toward patient goals.
- →Deliver skilled medical social work services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
- →Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
- →Collaborate with patients, caregivers, physicians, and the interdisciplinary team to promote safe, effective care and successful discharge planning.
- →Provide counseling, emotional support, caregiver education, crisis intervention, and referrals to community resources, financial assistance, transportation, Durable Medical Equipment, long-term care, and other support services.
- →Advocate for patients, address psychosocial barriers to care, and communicate changes in patient status to the care team.
- Master’s degree in social work (MSW) from an accredited program.
- Current Michigan Social Work license (LMSW or LLMSW) in good standing.
- Home health or medical social work experience preferred.
- Strong assessment, critical thinking, crisis intervention, and problem-solving skills.
- Excellent communication, counseling, and interpersonal skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Reliable transportation, a valid driver's license, and current auto insurance.
