At Home Care is seeking a Home Care Nurse who will complete the nursing process, collaborate with physicians and multidisciplinary team members, provide physical and psychological support to patients, their families, and friends, and supervise assigned team members. The LPN or RN will report directly to the Director of Nursing. 


Skills/Qualifications: An LPN or RN must be 18 years of age with a high school diploma and a Certificate of Practical Nursing and/or an Associate's Degree, and have a current unrestricted license to work in the state of Missouri and be CPR certified. 


Job Responsibilities:

  • Provide skilled nursing interventions to treat the patient’s illness, address their rehabilitative needs, and promote preventive care. 
  • Utilize a skilled approach in the provision of problem-specific therapeutic interventions, teachings, and training activities following the plan of care. 
  • Explaining services by performing community outreach. They will educate clients, hospital staff, physicians, and community stakeholders on Home Care Services and the admission process. 
  • Promotes and restores patients' health by completing the nursing process, collaborating with physicians and multidisciplinary team members, providing physical and psychological support to patients, their families, and friends, and supervising assigned team members.
  • Confer with the RN supervisor regarding any necessary changes to the Plan of Treatment/Care.
  • Provide nursing assessments and patient education. 
  • Treat wounds, change wound dressings, give injections, and administer enemas.
  • Collect specimens from patients.
  • Administers medications and sets up medication dispensers.
  • Measures and records vital signs.
  • Reports and observed changes in patient conditions to the Director of Nursing.
