The Alzheimer Society of Peterborough, Kawartha Lakes, Northumberland and Haliburton is seeking a compassionate, skilled professional to join our team as a First Link Care Navigator.
In this role, you will make a meaningful difference in the lives of individuals living with dementia and their care partners. You will build trusted relationships, support people through complex systems, and help promote early intervention, well-being, and quality of life.
About the Role
As a First Link Care Navigator, you will provide intensive case management and guide individuals and families through the assessment and diagnosis process. You will offer supportive counselling, coordinate participation in Alzheimer Society programs, connect clients to services, and collaborate with community partners to ensure no one navigates dementia alone.
Key Responsibilities
- Provide person-centred supportive services including individual and family counselling, dementia education, advocacy, future care planning, and system navigation
- Manage a caseload primarily consisting of clients with complex needs
- Conduct assessments using the First Link Standardized Assessment and develop individualized support plans
- Support clients through pathways for assessment and diagnosis
- Support individuals living with dementia to remain safely in their homes as long as possible
- Provide education on dementia care practices to promote safety, reduce care partner stress, and prevent crisis situations
- Support care partners in responding to behavioural expressions of dementia
- Attend memory clinics, to conduct care partner interviews and participate in inter-professional team meetings to assist in the diagnostic process and develop plans for support.
- Build and maintain strong relationships with community partners
- Represent the Alzheimer Society on committees and community initiatives
- Deliver services across multiple settings (office, home, community, clinic, virtual, phone, and email)

Qualifications
- Diploma or degree in Social Work, Social Services, Geriatric Care, or a related field (or equivalent experience)
- Preference given to regulated professionals in good standing with an Ontario regulatory body
Experience
- Supporting individuals living with dementia and their care partners
- Assessment, care planning, and service coordination
- Working in healthcare, human services, or inter-professional environments
Skills & Competencies
- Strong knowledge of dementia care and person-centred practices
- Excellent interpersonal and communication skills
- Strong organizational and time-management abilities
- Cultural sensitivity and awareness of diversity
- Knowledge of local health and social service systems
- Proficiency in Microsoft Office and client databases
Additional Requirements
- Valid G Class driver’s license and access to a reliable, insured vehicle
- Vulnerable Sector Police Check (within the last 2 months)
- Membership in a relevant regulatory body is an asset
Our Compensation Package
- Grade 4 position with starting salary commensurate to experience: $24.00 - 29.00 per hour (35 hours per week)
- Competitive platinum level benefits package that rivals what is offered at many large corporations, fully covered by the Society* and includes Employee Assistance Program (*excludes LTD)
- 15 days vacation, in addition to office closure between December 25 and January 1. All vacation hours are accessible to you within three months of your employment with us.
- 18 sick days per year, 5 of which may be used for family obligation reasons
- 5 paid and 5 additional unpaid emergency leave days per year
- 1 Wellness Day after five years of service
- In the summer months, flex time may be used to take Friday afternoons off making the most of the great weather and everything our four counties have to offer.
