August Rose Health Center’s
community-based psychiatric rehabilitation program (PRP) offers children and adults with mental illness a range of rehabilitation services that focus on emotional well-being, recovery, and resource connection including mental health service connection, housing, and entitlements coordination. The PRP programs services are person-centered, strength-based, and recovery-oriented, and are provided at a frequency and intensity that meets each client s individual needs. The PRP program is CARF accredited and designed to facilitate independence and community engagement, using leading-edge and evidence-based practices that consider each person's strengths, needs, desires, abilities, health, and safety.


Position Summary:

The Psychiatric Rehabilitation Program Counselor works to assist individuals with mental illness to reach a higher level of independence. Services which are provided are individualized and based upon a detailed needs assessment that is conducted on each consumer. The PRP Counselor provides support with accessing and coordinating benefits, services and community-based resources. PRP services are provided on-site, within the home, and in the community. The consumer receives assistance with learning and developing improved daily living skills (i.e. personal hygiene, organizational skills, time management skills, nutrition, and money management), as well as working towards achieving healthy coping mechanisms.


Responsibilities


PRP Counselor will facilitate services at a minimum of 4 contacts (for children & adolescents) per month. Services will be a minimum of 30 minutes per contact.


PRP Counselor will work and collaborate with families and professionals to coordinate services with other agencies and programs in the best interest of the individual.


PRP Counselor is responsible for the development and annual review of individual rehabilitation plans, DLA-20 assessments, and individual safety plans.


PRP Counselor will document all contact, interaction, and interventions including dates, locations, and types of contact.


PRP Counselor will summarize the client’s progress monthly regarding:


  • Goal progress, regression, and other achievements
  • If applicable, changes in rehabilitation goals or services requested

  • Referral to and collaboration with external resources and providers


PRP Counselors assist clients with working on their individualized rehabilitation plan goals in the following areas to assist the client with the restoration of skills:


  • 8 Dimensions of wellness – Social, Occupational, intellectual, financial, environmental, physical, spiritual, and emotional

  • Self-care skills--hygiene, nutrition, self-administration, and compliance with medication

  • Social skills--community integration activities, interactive skills with peers and authority figures, developing natural support, and supporting the child's participation in community activities

  • Independent living skills--maintenance of personal living space, community awareness, mobility skills, money management, and accessing available entitlements

  • Health and wellness promotion and training in nutrition, exercise, and dental care

  • Relapse prevention education in the home and in the community

  • Entitlements coordination

  • Facilitate the client’s participation in the monthly on-site group meetings to encourage peer socialization and the development of coping strategies/techniques.


Knowledge, Skills, and Abilities



  • Experience in behavioral health assessments, treatment/discharge planning, and evaluation of outcome-based system measures

  • Extensive knowledge of community-based resources that support the emotional health and recovery of clients served

  • Professional communication and collaboration skills and relationships across disciplines, settings, and situations

  • Experience with independently taking on new tasks and responsibilities in a proactive way

  • Ability to interact in a virtual environment

  • Strong writing skills and verbal communication skills

  • Strong organizational and time management skills

  • Strong problem-solving skills

  • Effective computer skills. Proficient in using Microsoft Office Suite Word, Excel, Outlook, and PowerPoint


Minimum Requirements:


  • CPR/First Aid certification

  • One year or more of experience working with children and adults in a community setting

  • Excellent written and oral communication skills

  • Strong time-management and organizational skills,

  • Computer literacy

  • Knowledge of community resources

  • Ability to work independently and on a team

  • Reliable transportation and a valid driver's license
