About Us
We are a growing private duty home care agency dedicated to helping seniors and adults remain safe, independent, and comfortable in their own homes. We are seeking an experienced Registered Nurse who is passionate about quality care, caregiver education, and community partnerships.

Position Summary
This part-time leadership role serves as the agency's Clinical Trainer and Alternate Administrator while supporting client assessments, supervisory visits, quality assurance, and community outreach.

Responsibilities
  • Clinical Leadership 
    • Serve as the agency's Clinical Trainer by providing orientation, education, competency evaluations, and ongoing training for caregivers.
    • Promote best practices in dementia and Alzheimer's care, fall prevention, infection control, client safety, and person-centered care.
    • Provide coaching and support to caregivers while maintaining high standards of service delivery.
  • Alternate Administrator
    • Serve as Alternate Administrator in accordance with agency policies and applicable state regulations.
    • Assist with oversight of daily operations in the Administrator's absence.
    • Support compliance with agency policies, documentation standards, and licensing requirements.
    • Participate in audits, caregiver supervision, and quality improvement initiatives.
  • Client Assessments & Supervisory Visits
    • Complete comprehensive in-home client assessments.
    • Develop individualized service plans based on client needs.
    • Conduct supervisory visits and reassessments to ensure quality care and client satisfaction. 
    • Collaborate with clients, families, caregivers, and healthcare providers regarding changes in client condition or service needs.
    • Maintain accurate and timely documentation.
  • Community Outreach & Business Development 
    • Develop and maintain relationships with physicians, hospitals, rehabilitation centers, skilled nursing facilities, assisted living communities, hospice providers, social workers, discharge planners, geriatric care managers, and other healthcare professionals.
    • Educate referral sources on the benefits of private non-medical home care services.
    • Identify opportunities to expand referral partnerships and support agency growth.
    • Represent the agency at networking events, community outreach activities, and healthcare meetings.
    • Maintain regular communication with referral partners to strengthen long-term professional relationships.
Qualifications
  • Current, unrestricted Texas Registered Nurse (RN) license.
  • Minimum of two (2) years of nursing experience.
  • Minimum of one (1) year of supervisory or management experience in a home health, hospice, or long-term care setting.
  • Experience training, supervising, and mentoring caregivers.
  • Strong clinical assessment and critical thinking skills.
  • Demonstrated experience caring for individuals living with dementia and Alzheimer's disease.
  • Excellent communication and interpersonal skills.
  • Proven ability to build professional relationships and network within the healthcare community.
  • Valid Texas driver's license, reliable transportation, and current automobile insurance.
  • Willingness to travel throughout Montgomery, Navasota, Willis, Pinehurst, and Magnolia.
  • Strong organizational and time-management skills.
  • Proficiency with electronic documentation and Microsoft Office.
Preferred Qualifications
  • Experience in private duty home care.
  • Knowledge of Texas home care regulations and standards.
  • Existing professional relationships within the local healthcare community are a plus.
