• →Complete OASIS Start of Care (SOC) admissions visits for home health patients throughout designated territories.
• →Perform comprehensive physical therapy evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• →Develop individualized treatment plans based on patient needs and physician orders.
• →Collaborate with nursing and interdisciplinary team members to coordinate patient care.
• →Educate patients, families, and caregivers to promote independence, safety, and optimal outcomes.
• →Accurately document clinical findings and services using electronic medical records and tablet technology.
• →Continuously evaluate patient progress and adjust care plans as appropriate.

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent experience as a Physical Therapist.
• Prior Home Health experience required.
• OASIS experience required.
• HCHB experience strongly preferred.
• Strong clinical, communication, and organizational skills.
