At BAYADA Home Health, you'll have the time to provide the compassionate, one-on-one care your patients deserve. As an Admissions RN, you'll play a vital role in welcoming patients into home health services by completing comprehensive Start of Care assessments and developing individualized care plans that set them up for success.

As an Admissions Registered Nurse, you will:

• →Complete 1–3 Start of Care (SOC) visits each day throughout Monmouth, Middlesex, Somerset, and Union Counties
• →Conduct comprehensive patient assessments and accurately complete OASIS documentation
• →Develop individualized plans of care in collaboration with physicians and the interdisciplinary care team
• →Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education
• →Educate patients and caregivers on disease management, medications, and home safety
• →Monitor patient conditions and communicate changes appropriately
• →Ensure documentation is accurate, timely, and compliant with regulatory standards

• Current Registered Nurse (RN) license in the State of New Jersey
• Minimum of one year of nursing experience (Home Health experience preferred)
• Experience completing OASIS documentation and Start of Care visits strongly preferred
• Comfortable using electronic medical records (EMR); Homecare Homebase (HCHB) experience is a plus
• Excellent assessment, communication, and time management skills
• Passion for providing exceptional, patient-centered home health care

Base Pay, depending upon qualifications, status, and subject to negotiation: 

$65.00-$75.00 per point ($101,400- $117,000 annual earning potential)
