RN Care Manager
Office location: Tinton Falls, NJ
Full-TimeSalary: $42-$50/hour based on experience and qualifications

Elder Life Management, now part of the BAYADA family, is seeking a compassionate, adaptable, and clinically strong Registered Nurse (RN) Care Manager to support older adults and their families in Tinton Falls, NJ with clients primarily in central jersey.

This is a unique opportunity for an RN who wants to use their clinical judgment in a more independent, relationship-driven role. As an RN Care Manager, you will work directly with older adults and their families to assess needs, develop individualized plans, coordinate services, advocate across settings, and help clients remain as safe, supported, and independent as possible.

This role is ideal for a nurse who enjoys meaningful one-on-one relationships, problem-solving, home visits, and guiding families through complex and changing situations. It is not a shift-based bedside role, and it is not a traditional office-based clinical manager role. It is a community-based nursing role that requires flexibility, maturity, sound judgment, and the ability to pivot as client needs change.

  • High-impact, relationship-based work with older adults and families
  • Meaningful autonomy with strong support from leadership
  • A chance to combine nursing assessment, care coordination, and advocacy in one role
  • Variety in your day across home visits, family support, provider coordination, and documentation
  • Afterhours and over time pay available.
  • Opportunity for additional compensation through after-hours billable work, when applicable
  • Agency car provided for work-related travel

  • Coordinate home care services and other service systems to support safety, well-being, and quality of life
