Speech Language Pathologist - SLP Home Health

You don’t need prior home health experience to apply—if you’re passionate about patient care, we’ll support you every step of the way.

In this role, you’ll be part of a collaborative care team, helping clients improve their communication, swallowing, and overall quality of life while allowing them to remain safely at home.

• →Visit clients in their homes across your assigned area
• →Evaluate and treat speech, language, communication, and swallowing challenges
• →Create personalized care plans and adjust them as your clients progress
• →Partner with other clinicians to deliver well-rounded care
• →Educate patients and families so they feel confident continuing care at home
• →Document your visits and progress using our easy-to-use EMR system

• An active SLP license in New Hampshire
• Graduate of a speech-language pathologist program accredited by the Council on Academic Accreditation in 
  Audiology and Speech-Language Pathology with the American Speech-Language Hearing Association 
  (ASHA).
• Comfort with basic technology and documentation systems

 Base Pay, depending on qualifications and subject to negotiations

$64.00-$70.00 per point
