Position Summary

Bloom Healthcare is also a growing population health company and national leader in accountable care performance, home-based medicine, and healthcare logistics innovation.  

We are currently looking for field-based Population Health Advanced Practice Provider to join our Team! 

Core Responsibilities: 

  • Complete patient visits across Bloom territory with focus on gaps closure, discovery of previously unidentified complex conditions 

  • Use technology enabled chart discovery to highlight clinical gaps and improve outcomes 

  • Support teams with HEDIS gap closure, STAR rating success 

  • Demonstrate exceptional communication and relationship-building skills with care team members, including internal clinicians and external partners 

  • With support from Sr Director of Population Health, help identify opportunities and execute process improvement plans to improve population health outcomes 

  • Engage with and appropriately utilize population health data to drive superior patient outcomes at the practice level 

  • Attend ongoing educational opportunities 

  • Field Pop Health Provider will not hold their own patient panel, but will complete visits on others’ Bloom Primary Care patients.  

Advanced Practice Provider Benefits: 

  • Scheduled Days: Monday - Friday (5 days), potentially flexible 

  • 10% remote, 90% in person 

  • For visits more than 1 hour from provider’s home, accommodation will be provided 

  • $10,000-$15,000 annual quality incentive opportunity

Preferred Qualifications: 

  • Master's degree required (Nurse Practitioner or Physician’s Assistant) 

  • Licensure in the state of Colorado 

  • 3+ years of clinical provider experience 

  • Strong knowledge of ICD 10 coding, documentation best-practices, and risk adjustment 

  • Experience and with HEDIS, quality measures for the MA population 

  • Advanced proficiency in principles of population health and value-based care 

  • Ideal candidate is flexible, loves working in a team, self-motivated, and has excellent communication skills.
     

The salary for this position is competitive and commensurate with experience. The compensation pay range for this role in the state of Colorado typically falls between $130,000 - $150,000 annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
