Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.

We are seeking a Home Care Occupational Therapist who will follow established policies and procedures to evaluate and treat a caseload of patients with impaired functional ability in their place of residence the HH OT will have accountability for the coordination of care, implementation of physician orders, and provision of care from admission to discharge.

Schedule:

Monday - Friday; 8:30am - 5:00pm

San Francisco & Marin Counties

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

Evaluates adult, and geriatric patients. Interprets findings to determine the nature and extent of dysfunction
Collaborates with physician, patient, and team members to establish, implement, and evaluate a plan of care including goals and periodically revises patient’s treatment/ care plan
Obtains required physician orders for all care provided. Provides comprehensive ongoing assessment of the patient and the patient’s equipment needs
Counsel patient/caregiver in meeting patient’s needs and goals. Notifies the patient’s attending physician and other appropriate healthcare professionals and staff members of significant changes in the patient’s condition in a timely manner
  • Bachelor’s Degree (B.S.) in occupational therapy or equivalent; Master's Degree preferred
  • One year's experience in a health care setting preferred

•    Current California license in Occupational Therapy
•    BLS required

Pay Range:
$58.00 - $73.60 hourly
