Capital City Nurses, a Care Advantage Company, is seeking a dynamic and compassionate Registered Nurse (RN) Home Care Supervisor to provide clinical leadership and oversight throughout the Chevy Chase, Maryland and Washington, DC service area.

As a Home Care Supervisor, you'll bring these values to life every day for patients, families, caregivers, and teammates.

  • Conduct comprehensive patient assessments and admissions.
  • Introduce patients and families to available services and care expectations.
  • Establish a foundation for exceptional care and patient satisfaction.
  • Develop and oversee personalized plans of care.
  • Collaborate with physicians, families, and caregivers to ensure goals are met.
  • Adjust care plans based on changing patient needs.
  • Monitor patient progress and clinical outcomes.
  • Communicate effectively with physicians and interdisciplinary team members.
  • Ensure continuity and quality of care.
  • Educate and support family members participating in patient care.
  • Promote patient safety, confidence, and successful care at home.
  • Provide clinical guidance and support to caregivers, aides, and nursing staff.
  • Reinforce best practices and company standards.
  • Review documentation and caregiver performance.
  • Provide feedback, coaching, and support to enhance quality outcomes.
  • Conduct supervisory visits to observe care delivery.
  • Ensure patient satisfaction, compliance, and service excellence.
  • Support orientation and ongoing education initiatives.
  • Promote continuous learning and professional development.
  • Ensure compliance with Maryland, District of Columbia, and applicable regulatory requirements.
  • Maintain adherence to company policies and clinical standards.
  • Complete and maintain accurate patient records and nursing documentation.
  • Ensure timely and compliant reporting.
  • Participate in quality initiatives and interdisciplinary team meetings.
  • Help drive continuous improvement in patient outcomes and service delivery.
  • Build and maintain strong relationships with referral sources, healthcare professionals, and community partners.
  • Represent Capital City Nurses and Care Advantage professionally throughout the community.
  • Support business growth initiatives while maintaining exceptional standards of care.

Requirements

  • Active Registered Nurse (RN) license in Maryland and/or Washington, DC (Compact License accepted where applicable)
  • Associate Degree in Nursing (ADN) or higher
  • Minimum of one (1) year of nursing experience
  • Strong leadership, communication, and interpersonal skills
  • Ability to work independently and collaboratively
  • Reliable transportation and willingness to travel throughout the service territory
  • Proficiency with electronic documentation and technology
  • Home care or home health experience
  • Case management experience
  • Supervisory or leadership experience
  • Experience conducting patient assessments and developing care plans
