Responsibilities

Responsible for screening uninsured patients for potential insurance coverage and to connect eligible patients to a Certified Enrollment Counselor.  Also helps support whole patient care of all patients by scheduling in-reach appointments for needed medical services.

 

  • Provide basic information and health education to groups and individuals in accordance with program guidelines and/or as directed
  • Provide health insurance information and education to patients, primary focusing on the uninsured
  • Assist patients to make appointments for new or renewal applications
  • Instruct patients on the documents necessary for the eligibility process
  • Assist patients to review their medical chart for needed clinical appointments
  • Conduct basic assessments/screenings of target population
  • Complete data collection forms
  • Compile and submit verbal and written reports
  • Demonstrate familiarity with VCC programs and services
  • Assist in developing a wide range of educational materials
  • Attend meetings, as needed
  • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
  • Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization

Requirements

  • High school graduate or equivalent
  • Bilingual English/Spanish
  • Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
  • Required to work flexible hours, including some evening and weekend hours
  • Strong verbal and written skills necessary for communication with patients/clients, clinicians and other staff
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
  • Ability to handle confidential materials and information in a professional manner
  • Excellent customer service skills and commitment to providing the highest level of customer satisfaction
  • Ability and willingness to meet the organization’s attendance and dress code policies
  • Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and Excel
  • $21.00 - $23.00 Depending On Experience
