Adult Family Care Case Manager

The Case Manager will coordinate, provide and supervise high quality supports to the individuals, families and providers served by Adult Family Care (AFC).  The Case Manager provides assistance with other agencies ensuring families are utilizing all available resources.

• →Act as program liaison to the Executive Office of elder Affairs, State agency Area Offices (DDS, MCB), and other Department personnel as delineated by Program Director.
• →Act as liaison with other agencies involved in providing services to the people we support, such as Day Programs.
• →Ensure that the program meets and complies with all applicable Medicaid regulations; oversee compliance of providers with mandated trainings.
• →Process participant referrals and the coordination of the admission process, as identified in State Agencies’ regulations regarding "placement services”.
• →Participate in recruitment, selection, and evaluation of Adult Family Care families. Interview families and facilitate the matching and transition of participants into homes.
• →Develop positive relationships with participants, families and monitor/assess placements minimally on a regular basis.
• →Train and supervise families in all facets of their responsibilities.
• →Support the development and growth of all participants served in the program.
• →Work with the AFC Registered Nurse as part of the multidisciplinary team.
• →Identify, monitor and complete certain section of the MDS/HC assessment, as well as a psychosocial assessment.
• →Ensure and maintain that all program record keeping, documentation and reviews are accurate and up to date; daily log, monthly case manager notes, home inspections, site safety plans, health record, psychosocial assessment etc..  Develop and complete part of the participant’s Plan of Care and annual MDS assessment.

• Bachelor’s Degree in related field with four years’ experience; or Master’s Degree and two years of experience.
• Ability to communicate effectively verbally and in writing.
• Intermediate computer skills and knowledge of Microsoft Office products.
• Ability to develop and maintain satisfactory working relationships with employees and providers.
• High energy level, superior interpersonal skills and ability to function in a team atmosphere.
• Must hold a valid driver's license and have access to an operational and insured vehicle.
• Demonstrated understanding of and competence in serving diverse populations.
