Patient Assessment and Care: Conduct thorough initial patient assessments, including health history, physical examinations, and symptom evaluation. Monitor and manage patient conditions, addressing acute and chronic illnesses, and collaborating with physicians on treatment plans.

Care Coordination: Coordinate patient care activities, including referrals to specialists, diagnostic tests, and follow-up appointments. Ensure seamless transitions between healthcare settings and maintain communication with interdisciplinary team members.

Documentation and Record Keeping: Maintain accurate and detailed patient records, including medical histories, symptoms, treatments, and outcomes. Adhere to regulatory requirements and organizational policies for documentation and reporting.

Patient Advocacy: Advocate for patients' rights and preferences, ensuring their voices are heard in healthcare decision-making processes. Serve as a liaison between patients, families, and healthcare providers to facilitate open communication and shared decision-making.

Requirements

Licensure: Current, unrestricted RN license from any state in the United States.
Certification: Current Basic Life Support (BLS) certificate accredited by the American Heart Association (AHA).
Experience: Minimum of four (4) years of current nursing experience to include at least two (2) years of inpatient experience in care coordination and at least two (2) years of nursing experience in an outpatient setting.
Education: ASN accepted; BSN preferred.
Education must have been completed at a school accredited by the ACEN or CCNE.
