We are looking for Direct Support Professionals that are passionate about providing quality care in a positive and empathetic manner. Our Direct Support Professionals must have a strong desire to work with autistic, intellectually and/or developmentally disabled individuals.
Responsibilities
~1 min readThe Service Coordinator manages assigned caseloads to ensure individuals have timely access to appropriate services, funding, benefits, and resources. This role oversees service authorizations, re-authorizations, eligibility and benefits, insurance issues, and funding for medical equipment and adaptive supports while ensuring compliance with applicable regulations and requirements.
The Case Coordinator collaborates closely with individuals, families, guardians, residential teams, treatment teams, external case managers, funders, and internal departments to coordinate services, resolve barriers, and support continuity of care. Responsibilities include maintaining accurate electronic records and documentation, managing service utilization, coordinating admissions and transitions, and ensuring annual plans and required documentation are completed accurately and on time.
The position also serves as a resource to program managers and staff, provides training as needed, and assists with resolving issues that may affect an individual's access to services, health, safety, and overall well-being. The Case Coordinator is expected to maintain strong knowledge of funding requirements, Bancroft policies and procedures, and applicable state regulations while demonstrating professionalism, confidentiality, ethical conduct, collaboration, and respect for the dignity and rights of those served.

Effective and proven ability to prioritize, multi-task; management skills required, as well as flexibility and adaptability. Ability to assess situations, problem solve and delegate tasks. Effective interpersonal and organizational skills. Basic math skills required. Operative knowledge of MS Office Word and Excel spreadsheets preferred. Valid driver’s license required in incumbent’s legal name and current address with no provisional restrictions. Minimum 18 years of age required.
