Case Manager - Temporary, Part-time

As a Case Manager-RN you are responsible for facilitating resident admissions and discharges, ensuring seamless transitions through their journey. You will work with families, residents as well as internal/external partners to ensure residents receive high quality care and services.

• →The RN conducts pre-admission assessment of prospective residents.
• →The RN develops and ensures continuity of dialogue with hospitals/social workers in community and outlying services regarding availability within community.

• Current state licensure as Registered Nurse
• Previous admission/discharge experience preferred
• Be able to read, write, and communicate in English
