~1 min read- Establish effective communication and referral pathways for individuals who are at risk of homelessness and in contact with other health and care agencies.
- Provide wrap around support in emergency/temporary/supported accommodations across the area.
- Prioritise caseload depending on crisis intervention and severity of needs.
- Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aid.
- Work closely with the local authority, Health & Social care sector, partners and any provision linked to preventing Homelessness and improve joint working.
- Increase and enhance partnership working with primary care, mental health services, housing and other voluntary organisations who are supporting individuals at risk of losing tenancies.
- Provide wrap around support on budgeting, education, training and employment whilst continuing to support their engagement in structured services.
- An understanding and knowledge of the harmful effects associated with drug and alcohol use in relation to health, social welfare, housing, employment and personal relationships.
- Minimum of 12 months experience facilitating successful tenancy sustainment and working to achieve positive community connections.
- Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs.
- The ability to work with and manage challenging behaviour.
- Have a valid UK driving licence, and use of own car, to work flexibly across different sites as required.
- 25 days holiday (+ bank holidays) rising by 1 day for each years’ service “Capped at 30 days”
