Responsibilities

→Experience of engaging and providing meaningful support to individuals with severe multiple disadvantages.
→Experience of communicating effectively with people requiring support
→
Experience of recording relevant sensitive information in a timely manner
→Knowledge of at drugs and alcohol support and at least one of the following; housing; homelessness; the benefits system, mental or physical health, the criminal justice system, domestic abuse
→ Experience of supporting people who may be affected by homelessness, mental health and substance use.
→
Knowledge of Safeguarding Procedures and their application.
→Experience of working with people in crisis
→Knowledge and experience of delivering trauma informed support
→To be able
- →To work as part of a multi-disciplinary team
- →To have excellent Interpersonal Skills / Ability to build rapport
- →To work independently; adhering to local risk management policies
- →To problem solve in challenging situations
- →To be service user focused, and service user led in support planning.
- →To walk for several hours each day to complete the outreach activity
- →To manage individuals that have multiple unmet needs and take support to the individual
- →Full driving licence and willingness to drive the Outreach Van
