Damar’s service and support array has expanded over the years, providing a full continuum of care including Applied Behavior Analysis (ABA) clinics, residential and group home operations for children and adults, intensive behavioral interventions, diagnostic and evaluation services, community-based services for child welfare and juvenile justice involved children and families, and primary and outpatient behavioral healthcare services, serving more than 1,500 individuals each day. Damar has a great opportunity for an Advanced Practice Provider to be part of a multi-disciplinary team, in an integrated care setting that offers complete patient care, and the opportunity to collaborate with psychiatrists, clinicians, physicians, nursing staff, and community health workers. The Advanced Practice Provider is responsible for delivering high-quality healthcare to Damar Health Services' patients both at the health clinic and on the residential campus. This provider is comfortable assessing, diagnosing, and treating patients of all ages and abilities. The Advanced Practice Provider is also skilled at managing a wide-variety of medications and is comfortable with collaborating with both internal and external providers.
What We Offer
~1 min read- Ensures health-related procedures and related policies are followed accurately throughout all program lines within Damar Services, Inc
- Serves as a member of the interdisciplinary team and provides/receives clinical and medical information from Damar and Damar contracted physicians
- Maintains on-call availability as needed
- Assesses, diagnoses, and treats individuals of all ages at both Damar Health Services and on Damar’s residential campus
- Uses a full range of therapeutic skills including the prescription of psychotropic medications to treat individuals, including those with intellectual and/or developmental disabilities
- Provides primary care, medication management, and consultation services to both Damar residential and Damar Health Services patients
- Orders, interprets, and evaluates diagnostic tests
- Conducts training to health services staff as needed
- Establishes and maintains clinic hours to best meet the needs of Damar’s residential and Damar Health Services patients
- Shows initiative by identifying process improvement opportunities in order to provide the most relevant, compassionate, and up-to-date care to all Damar patients
- Collaborates with both internal and external medical providers in an effort to ensure patient continuity of care
- Takes pride and ownership in Damar’s facilities and assigned work areas
- Will adhere to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual
- Contributes to policy development, quality improvement, and practice evaluation
- Ensures clinical outcomes are met
- Oversees patient health records and ensures recordkeeping in compliance with HIPAA and other regulatory standards
- Adheres to all Damar policies and procedures
- Maintains adequate and timely documentation of patient health records
- Seeks the best use of materials, equipment and staff to maximize efficiency and effectiveness
- Ensures all activities, services, and programs through Damar are conducted in a manner that is sensitive to and shows respect for the cultural and ethnic diversity for all Damar patients
Requirements
~1 min readRequires MSN as a Nurse Practitioner or MPA and NCCPA certification as a Physician Assistant in the state of Indiana
Experience working with a variety of ages preferred
Experience in serving individuals with developmental disabilities, and knowledge of current trends and research in the use of psychotropic medications with individuals with developmental disabilities preferred
Certification as a Nurse Practitioner or Physician Assistant in the state of Indiana required
- Proficiency in medical instruments and equipment
CPR certification required
DEA registration for schedules II-V required
Trauma informed care and de-escalation training
Minimum age of 21 per licensure requirements
Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02 Pre-Employment and Employee Background Checks
Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving)
Successful completion of pre-employment physical required
