The Resource Specialist supports discharge throughput at a child and adolescent inpatient psychiatric hospital by coordinating the practical outpatient and community-based services needed for a safe and timely transition from the hospital. Working closely with Social Work and the treatment team, this role is primarily responsible for scheduling discharge appointments, securing aftercare follow-up, coordinating with outpatient providers and community agencies, tracking referral status, and identifying barriers that could delay discharge. The position also maintains current resource and provider contact lists, helps build and sustain working relationships with external providers and community partners, and supports efficient access to care by keeping referral pathways organized and up to date. By managing high-volume scheduling, referral, and tracking tasks and escalating unresolved barriers to the appropriate clinical staff, the Resource Specialist helps keep discharges moving while allowing licensed team members to remain focused on assessment, treatment, and other clinical responsibilities.

Responsibilities

  • Acts as an advocate on behalf of patients/families in obtaining needed services and/or resources for which they are eligible.
  • Updates information resource information directory and maintains related literature & applications for distribution to staff and families in specific area of concentration.
  • Documents services provided in patient’s medical record and applicable databases.
  • Negotiates and advocates access for patients to selected skilled, acute or long term care facilities. Identifies available transportation resources and negotiates with patient/family for cost effective, safe transportation.
  • Works with patient/family to assess patient eligibility for medication, co-pay and additional resource assistance.
  • Facilitates the patients/families application to public and private government and community agencies, primarily Medicaid.
  • Provides information and assistance to patients/families to secure financial and health assistance, organization financial assistance, and other governmental programs.

  • Requirements

    • Bachelor's degree in Social Work or Human Service field or the equivalent in education and experience required.
