Memory Care Coordinator

Wheatfield Commons, a DePaul Senior Living Community is hiring a full-time Memory Care Coordinator.
The pay range for this opportunity is $23.00 - 23.60 per hour.

Responsibilities
~1 min read- →Respects and maintains resident rights and confidentiality and demonstrates a caring, positive attitude toward all residents, staff, guests, and service providers.
- →Performs the duties and responsibilities of the Medication Coordinator as assigned.
- →Performs the duties and responsibilities of the Supervisor as assigned.
- →Assists in recruiting, screening, and interviewing resident care staff. Assists with the orientation for new resident care employees.
- →Assists in conducting in-service education and training programs for staff on a monthly basis.
- →Attends and participates in staff and team meetings.
- →Makes recommendations for assignments and promotions. Participates in coaching, corrective, and disciplinary personnel plans.
- →Assists with the development of all staffing schedules for the Resident / Memory Care Department.
- →Completes rounds daily, make recommendations for daily resident care assignments and ensures follow-through of resident care staff. Provides assistance to resident care staff when necessary.
- →Assists the Resident Care Director to ensure that all policies, procedures, and Department of Health regulations are adhered to in the department.
- →Consults with the Resident Care Director as needed concerning resident care and personnel concerns.
- →Assists with the admission, transfer and discharge of residents as needed.
- →Performs other duties as needed or requested by the Director and / or Administrator.
Requirements
~1 min read- Ability to sit, stand, and use a computer for extended periods. Must be able to lift to 20 lbs., communicate effectively, and perform basic office tasks such as typing, filing, and operating standard office equipment.
- Must be at least 18 years of age.
- High School Diploma or GED
- 3 years’ experience as a supervisor, working with the elderly, preferably with those that have dementia diagnosis.
- Well developed organizational skills.
- Ability to work effectively with inspectors, monitoring, and licensing agencies.
- Must have the required annual Tuberculosis (TB) testing and annual Health Assessment completed.
- Must have the required references and criminal record check.
- Must have a valid New York State driver’s license that meets agency’s clean driving record policy.
