Caremate Wellness Solutions is seeking an organized, dependable, and compassionate Community Care Coordinator I to support our clients and caregivers.
In this role, you will coordinate daily care services, monitor Electronic Visit Verification (EVV), maintain accurate documentation, assist caregivers, and help ensure compliance with Texas Health and Human Services Commission requirements and company policies.

Key Responsibilities
  • Monitor daily service delivery and help resolve scheduling or care-related concerns.
  • Identify scheduling conflicts and communicate them promptly to supervisors.
  • Optimize caregiver schedules and authorized service hours.
  • Maintain accurate EVV records in HHAeXchange+ before claim submission.
  • Train and assist caregivers with the EVV mobile application.
  • Review GPS data to verify service locations and help prevent fraud, waste, and abuse.
  • Complete and review visit notes, timesheets, chart audits, and other required documentation.
  • Enter and monitor service authorizations and provider-type changes.
  • Track authorization expiration dates and follow up with clients.
  • Create and maintain accurate client and caregiver records.
  • Assist with recruitment, onboarding, compliance reviews, and quality-improvement activities.
  • Maintain the confidentiality of all client, caregiver, and financial information.
Qualifications
  • High school diploma or GED required; college coursework preferred.
  • One year of office administration, direct care, or quality-improvement experience preferred.
  • Strong organizational, communication, and problem-solving skills.
  • Basic computer and office-equipment proficiency.
  • Ability to manage multiple priorities in a fast-paced environment.
  • EVV or HHAeXchange+ experience preferred.
  • Knowledge of Medicaid-funded home and community-based services is a plus.
Work Requirements
  • Primarily office-based position.
  • Flexible or extended hours may occasionally be required.
  • Occasional local travel may be required.
  • Ability to occasionally push or pull up to 50 pounds.
  • Ability to remain professional and courteous under pressure.
