Remote Nurse - Multilingual (English, Mandarin & Cantonese)

We are seeking a detail-oriented Remote Nurse fluent in English, Mandarin, and/or Cantonese to support Home Health patients and caregivers. Responsibilities include contacting caregivers to confirm patient visits, ensuring they have the correct plan of care, sending documentation when needed, and accurately documenting all interactions in the EMR system.

This position is remote and offers you the flexibility of working from home.

• →Contact caregivers prior to or during scheduled visits to confirm arrival at the patient’s home
• →Ensure caregivers have the current and appropriate patient Plan of Care
• →Email or securely transmit care plans and related documentation to caregivers when necessary
• →Document all communication, follow-ups, and care coordination activities accurately in the EMR system
• →Escalate patient care concerns, missed visits, or compliance issues to the appropriate clinical supervisor
• →Maintain HIPAA compliance and patient confidentiality at all times
• →Support continuity of care and timely coordination between field staff and office personnel
• →Assist with basic care coordination and administrative nursing tasks as assigned

• Registered Nurse with New York License - USRN - NY
• Fluency in Cantonese and/or Mandarin and English required
• Previous experience in Home Health, Care Coordination, Case Management, or Remote Nursing preferred
• Strong verbal and written communication skills
• Experience using EMR/EHR systems
• Excellent organizational and documentation skills
• Ability to work independently in a remote environment
• Proficient with email, phone communication, and basic computer systems

You will need to have some essential tools – a reliable computer and noise-canceling headset, a second monitor for enhanced productivity, and a stable internet connection. You’ll also be required to have a backup internet connection, ensuring that you’re well-equipped to complete your work seamlessly.
