The Scheduler is responsible for coordinating and managing patient visit schedules for a home health agency, ensuring efficient allocation of clinical staff while maintaining high-quality patient care. This role requires strong clinical understanding, excellent communication skills, and the ability to manage multiple priorities in a fast-paced environment.

This position is remote and offers you the flexibility of working from home.

Responsibilities

  • Develop, maintain, and adjust daily and weekly schedules for field clinicians based on patient needs, staff availability, and geographic considerations
  • Coordinate start-of-care visits, routine visits, and urgent scheduling changes
  • Communicate effectively with nurses, therapists, patients, and families to confirm appointments and resolve scheduling conflicts
  • Ensure compliance with agency policies, regulatory requirements, and documentation standards
  • Monitor staff productivity and optimize scheduling efficiency
  • Collaborate with clinical and administrative teams to support continuity of care
  • Maintain accurate scheduling records in the agency’s system

Requirements

  • Fluency in both Cantonese & Mandarin and English required
  • Minimum 1–2 years of experience in home health, care coordination, or clinical scheduling preferred
  • Strong organizational and time-management skills
  • Proficiency with scheduling software and electronic health records (EHR) systems
  • Excellent interpersonal, problem-solving, and multitasking abilities
