We are looking for a USRN for the role of Scheduler. The Scheduler is responsible for coordinating and managing patient visit schedules for a home health agency, ensuring efficient allocation of clinical staff while maintaining high-quality patient care. This role requires strong clinical understanding, excellent communication skills, and the ability to manage multiple priorities in a fast-paced environment.

This position is remote and offers you the flexibility of working from home.

Responsibilities

  • Contact caregivers prior to or during scheduled visits to confirm arrival at the patient’s home.
  • Ensure caregivers have the current and appropriate patient Plan of Care.
  • Email or securely transmit care plans and related documentation to caregivers when necessary.
  • Document all communication, follow-ups, and care coordination activities accurately in the EMR system.
  • Escalate patient care concerns, missed visits, or compliance issues to the appropriate clinical supervisor.
  • Maintain HIPAA compliance and patient confidentiality at all times.
  • Support continuity of care and timely coordination between field staff and office personnel.
  • Assist with basic care coordination and administrative nursing tasks as assigned.

Requirements

  • Fluency in both Spanish and English required
  • Minimum 1–2 years of experience in home health, care coordination, or clinical scheduling preferred
  • Strong organizational and time-management skills
  • Proficiency with scheduling software and electronic health records (EHR) systems
  • Excellent interpersonal, problem-solving, and multitasking abilities

You will need to have some essential tools – a reliable computer and noise-canceling headset, a second monitor for enhanced productivity, and a stable internet connection. You’ll also be required to have a backup internet connection, ensuring that you’re well-equipped to complete your work seamlessly.

US Eastern Standard Time (EST) - 9am to 6pm
