Summary:

Providing direct patient care coordination for diverse populations, in alignment with CHP network strategic goals and community partnerships. The Care Coordinator collaborates with providers (inpatient and outpatient), hospitals, home care agencies and additional community-based organizations to assure collaborative and consistent patient care delivery. Applicant should have strong personal, analytical, problem solving, critical thinking, organization and decision making skills. The Care Coordinator must possess the ability to function independently and have the flexibility and personal integrity to work in partnership with CHP network stakeholders and partners as assigned. Strong attention to detail with the ability to prioritize, coordinate, and simultaneously maintain multiple projects with high level of quality and productivity is a must.


Roles and Responsibilities:

  • Promote the mission, vision, and values of Cayuga Health Partners.
  • Actively participates in CHP trainings and activities as assigned.
  • Appropriately recognizes and responds to opportunities for improvement.
  • Positively and proactively interacts with team members and network partners.
  • Collects data as indicated to meet evaluation needs of the program.
  • Prioritizes care programs as assigned and routinely updates practice integration plans. Appropriately documents all patient encounters as indicated.
  • Maintains patient confidentiality in the provision of quality care.
  • Serves as a representative of CHP, CHS, and practices as assigned.
  • Participates in special projects in alignment with system and network goals.
  • Practices always within scope - activities may be carried out within the role provided you have the knowledge, skills, and experience to do so lawfully, safely and effectively.
  • Actively participates in organizational and community committees.

  • Standard salary rate of $27.88 - $36.00 per hour ($57,990.40 - $74,880.00 annually) with benefits
