CASE MANAGER - PATH PROGRAM

Join the Cumberland County Guidance Center as a Full-Time Case Manager with our PATH (Projects for Assistance in Transitioning from Homelessness) Program.

As a PATH Case Manager, you'll work directly with individuals experiencing homelessness and serious mental illness to help them obtain stable housing and supportive services.

Key responsibilities include:

• →Conducting community outreach and engagement activities
• →Developing Individual Recovery Plans (IRPs)
• →Providing ongoing case management and care coordination
• →Assisting clients with permanent, supportive, and emergency housing resources
• →Connecting individuals with mental health, medical, substance use, vocational, and educational services
• →Assisting with public benefits and entitlement programs
• →Utilizing Motivational Interviewing (MI) and recovery-oriented interventions
• →Maintaining accurate clinical documentation and compliance requirements
• →Supporting recovery and long-term independence

• Bachelor's Degree in Social Work or a related Human Services field
• Minimum two years of experience in fields such as:

  • Behavioral Health
  • Mental Health Services
  • Social Services
  • Homeless Services
  • Human Services
  • Community Outreach
• Knowledge of housing resources and public benefit programs
• Strong communication, advocacy, and relationship-building skills
• Valid driver's license with acceptable driving record

• Experience working with individuals experiencing homelessness or housing instability
• Experience supporting individuals with serious mental illness or co-occurring disorders
• Knowledge of Housing First and supportive housing models
• Experience with crisis intervention and care coordination
• Bilingual English/Spanish
• Experience utilizing Motivational Interviewing and trauma-informed care
