This is a temporary position. The length of the assignment is estimated to go from September 2026 through December 2026, but this is always dependent on business need, and these dates may change. While the assignment would be at the Alliance, you would be an employee of a temporary employment agency that we would connect you with if hired for this position. This is a full-time (40 hours/week) temporary assignment.
Reporting to the Care Management Director, this position:
- Provides direct Enhanced Care Management (ECM), Community Supports (CS), and transitions of care (TOC) support and care coordination for Alliance members to ensure timely connection to appropriate care and optimize utilization
- Monitors and audits ECM, CS, and TOC provider performance to ensure delivery of high-quality, non-duplicative services
- Coordinates with internal and external teams and members to support seamless transitions across the care management continuum
- Acts as a liaison between the ECM/CS Program and external providers, community agencies, and Alliance departments
- Assists in ECM/CS program implementation, oversight, reporting, and provider training to support compliance with regulatory requirements

- Knowledge of:
- The principles and practices of community-based nursing
- The principles and practices of community-based service delivery to at risk populations, including motivational interviewing, trauma informed care, and other best practices
- Community-based organization and/or public health practices and processes
- The principles and practices of case management
- Care management and coordination
- Ability to:
- Analyze, interpret and apply legal, regulatory and contractual language, policies, procedures and guidelines, and legislative and regulatory directives
- Evaluate medical records and other health care data
- Act as a technical resource and explain regulations, processes, and programs related to area of assignment
- Mange multiple projects simultaneously, organize work, and achieve goals and timelines
- Education and Experience:
- Current unrestricted license as a Registered Nurse issued by the State of California
- Associate's degree in nursing and a minimum of seven years of experience in outpatient medical, care management, community health service delivery, behavioral health practice management or related healthcare experience (a Bachelor's degree may substitute for two years of the required experience); or an equivalent combination of education and experience which would provide the required knowledge, skills and abilities may be qualifying
- We are in a hybrid work environment and we anticipate that the interview process will take place remotely via Microsoft Teams.
- While some staff may work full telecommuting schedules, attendance at quarterly company-wide events or department meetings will be expected.
- In-office or in-community presence may be required for some positions and is dependent on business need. Details about this can be reviewed during the interview process.
