The Clinical Nurse Manager is responsible for the clinical management of an assigned caseload of clients. The Clinical Nurse Manager will lead and supervise all field staff toward specific outcomes on assigned cases under their supervision. They will ensure compliance with all clinical standards, policies, and procedures, including, but not limited to performing case opens and assessments, medication reconciliation, creation of clinical documentation, supply inventory, and medical status updates for patient population. This role also maintains compliance with the accrediting agencies, state, and funding source requirements.

Success measures include adherence to Centria policies, flawless execution of tasks, and meeting or exceeding personal qualitative and quantitative key performance indicators (KPI) metrics.

Responsibilities

  1. Perform Case Opens.
  2. Monitor and proactively schedule clients for up to 15 in-home supervisory visits per week.
  3.  Prepare and process documentation related to performing visits.
  4. Communicate with field staff to build relationships, offer support, and obtain feedback on client status.
  5. Review shift notes to obtain recent history on client status and determine the quality of the record.
  6. Work closely with the Director of Clinical Services and the Healthcare Team to provide support for staff and clients.
  7. Provide a positive experience for staff and clients.
  8. Perform Telehealth On-Call duties on a rotating basis.
  9. Maintain client care plans including creating, sending, and tracking new/updated Plans of Care (POCs) and addendums.
  10. Provide clinical education to the client, family/guardians, and field staff members.
  11. Make administrative and procedural decisions and judgments on sensitive and confidential issues.
  12. Document and report on patient progress, challenges, complaints, incidents, and status changes.
  13. Manage inventory for supplies and medications on cases when necessary.

Requirements

  • Working knowledge of laptop/desktop PC
  • Proficiency in Microsoft Suite (Word, Excel)
  • Proficiency in G Suite (Gmail, Drive, Docs, Sheets, Google Meet)
  • Ability to follow written instructions
  • Ability to use computers and computer/software programs
  • Ability to communicate expressively and receptively
  • Bachelor’s degree required
  • Current Registered Nurse (RN) license in good standing.
  • Minimum 1+ year experience in nursing
  • Knowledge and/or experience in a home care model
  • Knowledge and/or experience with SCI, TBI, Respiratory failure, Pediatric Populations
  • Demonstrated ability to work with an interdisciplinary team (Operations, Clinical, and Field Staff);
  • Superior customer service skills;
  • Proven ability to effectively document findings of visit and communication with staff to meet objectives and paint a clear picture of client status;
  • Highly organized with the capacity to work under tight deadlines and shifting priorities;
  • Centria's office hours are Monday through Friday from 8:30 AM – 6:00 PM.
  • Travel is required to perform in-home visits.
  • Additional time or occasional shift in schedule may be required to complete the above work or meet company objectives.

While performing the duties of this job, physical requirements such as bending, reaching, lifting, pushing, or pulling up to 30 pounds may be required. This role will require sitting most of the day as well as walking and standing periodically. This role may require close visual acuity on computer screens or monitors and the ability to analyze data and figures on a screen.
