Home Health Nurse- LPN/ RN

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.

Our client is a 25 year old male who is non-mobile, stays in a hospital bed in the living room, and requires ongoing trach, ventilator, and g-tube management. He was diagnosed with Lennox-Gastaut Syndrome, a severe form of epilepsy characterized by multiple types of seizures that typically begin in early childhood. 

• →Clinical Care: Main responsibilities will include trach, ventilator, and g-tube management and he has a history of Lennox-Gastaut Syndrome.

• →Provide skilled nursing care (trach/vent care, medication administration, catheter care)
• →Monitor and document patient condition and vital signs
• →Communicate with families and care teams
• →Maintain safe, high-quality, patient-centered care

• Environment: Very clean, pet free home

We are looking to fill the following 6:00 AM – 6:00 PM and 6:00 PM – 6:00 AM shifts:

• Every day

• Full-time and part-time shifts available
• Day, night, and weekend shifts
• Weekly pay + optional daily pay
• Competitive pay based on experience
  
  

• Active RN or LPN license in the state of Michigan
• Home care or private duty experience preferred (not required)
• Trach/vent experience a plus (training provided)
• Strong communication and critical thinking skills
  
  

• Pediatrics (birth–21 years)
• Adults with complex medical needs
