- Provide exceptional physical therapy services to a diverse resident population
- Perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions to maximize residents' mobility, function, and quality of life.
- Collaborate with residents, families, and the interdisciplinary care team to support rehabilitation goals, coordinate care, and ensure safe, effective discharge planning and referrals.
