Position Summary: The Behavioral Health Consultant Associate serves as a member of the medical team providing direct care to the population of medical practice and working under the supervision of a licensed Behavioral Health Consultant. The BHA provides integrated behavioral health care during and after medical visits, by implementing the principles of Primary Care Behavioral Health. The BHA reports to the Behavioral Health Consultant.
Qualifications:
- Completed Master’s Degree in Social Work. Licensed Social Worker (LSW), or eligible in Pennsylvania.
- Clinical track and internship documentation required.
- Licenses and Certifications: Current State of Pennsylvania LSW Licensure, or LSW eligible.
- This position regularly serves Spanish- speaking patients and the ability to communicate professionally in Spanish is a plus.
Scope of Responsibilities:
- Provide behavioral health consultation services to individuals, families, and groups, as needed based on the needs of patients and medical practice.
- Work as a member of the healthcare team providing behavioral health services to pediatric and adult patients with behavioral health issues, substance use disorder and/or chronic medical conditions.
- Crisis assessment, intervention, and safety planning as needed
- Perform targeted strengths-based assessment and evaluation, including diagnostic impressions and functional status focused on the presenting problem.
- Provide timely and succinct feedback to Providers regarding consultation findings and recommendations.
- Concise documentation in the patient’s electronic medical record.
- Triage and refer patients to specialty mental health care when appropriate.
- Provide brief follow-up, including relapse-prevention education.
- Provide on-going consultation services for a sub-set of patients who require on-going monitoring and follow-up.
- Participate in weekly clinical supervision with the ChesPenn Behavioral Health Consultant.
- Abilities to work at other locations assigned.
- Complete training required by organization.
- Perform other duties as assigned.
Physical demands:
- Moderate physical effort in carrying charts, forms, supplies and equipment.
- Work environment:
- Primary work environment is a busy multi-cultural ambulatory care setting. There is routine exposure to infectious disease and blood borne pathogens.
