Integrated Services – Cold Lake First Nations
Integrated Services is seeking an Elder Wellness Worker to provide culturally grounded, relationship-based support to Elders in the community. This position focuses on supporting Elder wellbeing, independence, connection to community and culture, and access to services and resources.
Key Responsibilities
- Build respectful and trusting relationships with Elders.
- Conduct regular wellness check-ins, home visits, and community outreach.
- Assist Elders in accessing health, housing, transportation, social, cultural, and other community supports.
- Provide advocacy, referrals, service coordination, and follow-up.
- Plan and support Elder wellness, cultural, social, recreational, and community activities.
- Support opportunities for Elders to connect with culture, language, land, family, and younger generations.
- Work with families, Integrated Services programs, community partners, and service providers to coordinate supports.
- Identify and appropriately respond to concerns affecting an Elder’s safety or wellbeing.
- Maintain accurate and confidential program and client records.
- Support program planning, reporting, and other Elder Wellness initiatives.
Qualifications
- Diploma or certificate in Social Work, Human Services, Community Support, Gerontology, Indigenous Studies, Health and Wellness, or a related field is an asset. A combination of relevant education and experience will also be considered.
- Experience working with Elders, Indigenous communities, community wellness, health and social services, or related programming is preferred.
- Strong communication, relationship-building, organizational, and advocacy skills.
- Knowledge of Cold Lake First Nations community, culture, traditions, language, and local resources is a strong asset.
- Ability to maintain confidentiality and professional boundaries while working in a close-knit community.
- Valid Class 5 Driver’s License and satisfactory driver’s abstract.
- Criminal Record Check and Vulnerable Sector Check required.
