Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are now hiring a Full-Time Qualified Intellectual Disabilities Professional (QIDP) in Brooklyn, NY. The Qualified Intellectual Disability Professional assists the Executive Director in developing and maintaining individual service plans that meet the needs of people with disabilities and state and area program guidelines.
Starting pay is $23.00/ per hour
Responsibilities
- Develop individual service plans (ISP)/Person Centered Plans (PCP)
- Coordinate admission and discharge activities of participants
- Prepare monthly goal progress notes and billing justification forms
- Maintain and analyze all incident/accident reports
- Coordinate behavioral intervention plans, to include routine and emergency services
- Monitor medication procedures, administration, and documentation
- Monitor staff compliance with plan implementation and licensing compliance
- Evaluate staff performance
- Additional tasks and responsibilities may be assigned
- Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation
Minimum Requirements
- Bachelor’s degree and two years’ experience in implementing community-based support service for people with disabilities
- Valid driver’s license and good driving record
- Excellent organizational skills, and the ability to set priorities and follow through under time constraints
- Ability to effectively and professionally communicate with program participants, and other agencies, staff, and members of the community
- Ability to respond to crisis situations, exercise professional judgement, and use common sense
- Knowledge of required state operating and reporting guidelines under OPWDD
- Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
