Registered Nurse

• →Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
• →Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
• →Advise supervisor of all concerns and issues related to the provision of nursing services
• →Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
• →Conduct programmatic trainings including medication administration and delegation
• →Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
• →Train employees on the medical needs of assigned individuals
• →Monitor health and medical conditions of individuals and refer for medical services as needed
• →Monitor the use of medications through medication administration logs
• →Ensure health service supplies are available and replenished as needed
• →Ensure program documentation and billable records are completed accurately and timely
• →Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
• →Participate with individuals in community, social, and recreational activities
• →Participate in circles of support for personal futures planning
• →Follow strict OSHA procedures regarding blood borne pathogens
• →Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
• →Additional tasks and responsibilities may be assigned
• →Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing

• Bachelor’s Degree or equivalent experience in nursing field
• Minimum of one year of experience providing nursing services to individuals with disabilities
• Valid R.N. license
• Valid driver’s license with a satisfactory driving record
• Complete all state and agency required training per state guidelines
• Ability to calculate figures related to medication dosages
• Understanding and commitment to community-based support for persons with disabilities
• Ability to exercise good judgment and remain calm in crisis situations
• Ability to establish a comfortable and supportive relationship with an individual receiving supports
• Proficient with Microsoft Office
• Excellent verbal and written communication skills
• Ability to effectively present facts and recommendations
• Ability to effectively communicate with program participants, third-party agencies, staff and members of the community
• Excellent time management skills

• Work in office, residential programs, day programs, and community environments
• May be exposed to changing weather conditions when working outside
• Hours and weekly schedules may change due to business needs and may include evening and weekend hours
• May be required to be on-call in cooperation with other management staff
• Occasional overtime may be required
• Frequent lifting, stretching, and other physical exertion may be required
• May be required to lift or move 50+ pounds
• May occasionally assist with wheelchair transfer of non-ambulatory individuals
• May be required to transport individuals utilizing your own vehicle or company provided vehicles
• May be exposed to various medical conditions, communicable diseases, blood, body fluids, or tissues
